Here’s a list of items to bring when visiting our urgent care center:

  • Insurance card
  • Identification
  • List of current medications
  • List of past surgeries and medical conditions
  • List of any allergies
  • Completed registration form(s)

Insurance Information and How to Pay

Insurance: While we accept most forms of insurance, we encourage patients to check with our team prior to your visit to ensure that everything goes smoothly. As a participant in most managed care plans, we provide services according to your contracted insurance guidelines. This includes the collection of your co-pay at the time of service. Also, please bring the most recent copy of your insurance card to each visit.

Self-pay: Patients who do not carry insurance may also self-pay at the time of service. 

Payment options: For the convenience and security of our patients and staff, we kindly request that all copays, deductibles, self-pay and patient balances be made via credit card, debit card or Google Pay. Our office does not handle cash transactions. We appreciate your cooperation and thank you for your understanding!
